CSJA Online Supplies Order Form

This form is to submit orders for shirts, hats, jackets, and other materials for delivery at fall seminar. This allows us to submit an order in advance of fall seminar, and have the items to you before you start fall. Prices for items are listed next to the order line. This charge can either be paid at fall seminar, or deducted from your first check this fall. If you have any questions, contact Mark Culp, quartermaster and treasurer. Orders placed by July 1 guarantees the item for delivery at fall seminar. Orders submitted after that time may not arrive by fall seminar, and will require additional logistical issues (shipping or delivery) in order to get the order to you for your use this season.
