Please be prompt with your assessment of the event and of the judges as we have many shows this year, and will have numerous feedback sheets to read every weekend. Thank your for your prompt attention to this matter, and for providing this important information as part of the adjudication process. Use the tab button to move to the next field. I double tapes for your show or panel members on your show are required, they still need to be sent to the respective caption heads in a timely manner. This sheet is for the use of the Executive Director and Assistant Director in both the booking of shows, the fulfillment of contractual obligations with the association, and in dealing with members of our association. All of this information will be used in booking future shows with the host, and in hiring judges in the future within or for the association, including guest judges.

GENERAL INFORMATION

Judges and captions not assigned by CSJA for this show. Are any of these judges high enough caliber that you would consider recommending them for membership in our association? PRE-CONTEST

Did all judges check in and confirm their housing needs with the chief judge at the proper time, maintaining and returning communication with the chief judge regarding any specifics that required replies or other information provided in a timely manner? If concerns are present, please list judge and describe the situation as it occurred. If none, type none and move on to next question.





THE CONTEST DAY

Were all judges prompt and properly dressed for the show? Did all CSJA assigned judges behave in the appropriate manner, following all school and district rules, including the smoking ban present on most school grounds? Please note names of individual(s) and area(s) of concern below. If none, type none and move on to next question.



Did any of the judges seem to have actual adjudication problems with any portion of the adjudicated event (numbers management, critique issues, scoring issues, tape issues, other issues not listed). Please note names of individual(s) and area(s) of concern below. If none, type none and move on to next question.





HOUSING ISSUES

Where there any concerns with the housing arrangements as made by the host for the judges? Was the hotel of an acceptable level for the judges? Where the arrangements made in advance and communicated to the chief judge in a timely manner? Did the host have reservations made and room arrangements communicated to the chief judge in a timely manner? Did any problem (host-associated) regarding housing occur during the weekend? Please include here any other concerns regarding the provided housing. If none, type none and move on to next question.



DECORUM IN DEALINGS

Did any of the judges (members, guests or other) cause an issue regarding housing, transportation or other areas? For example, did a judge contact the director or sponsor directly to arrange for housing and transportation rather than go through the chief judge as is the policy of the association? Did any judge ask for special accommodations or special rooming desires that are not a part of the normal housing situation for the association? (If a judge booked their own accommodations and let you know that they would not be needing housing, this is not a concern of the association. However, if the judge booked accommodations after saying they would take provided housing, and cost the sponsor money, please note that below). Also, note any other concerns regarding panel members dealings with the sponsor or the director without the chief judge's consent or knowledge in the area below. If none, type none and move on to next question.





TAPE ISSUES


The chief judge should take time during the evening to insure that all tapes for all judges are working correctly. Many double tapes are arriving to caption heads that are either blank, or have so many flaws in recording that the tapes are impossible to understand. The chief judge needs to check during the show to insure that all the required double tapes are being made, and that the tapes that are being made are clear and understandable. Please note any concerns regarding tape issues here. If none, type none and move on to next question.





HOST CONCERNS

Please note any other area of concern or situations that arose that required your attention or the panel to make exceptional changes to make the show happen. For example, if sheets were not prepared, or tapes were not prepared and available before the show, that would be an issue that it is important to note. If the tabulation was done poorly or incorrectly, or if errors on the tote sheet are substantial, that would also be an area of concern. If the panel worked the first five bands without either sheets or tapes (using personal materials), make note of that. If any other issue arose during the event, please enter it below. If none, type none and move on to next question.



Did the host have the check prepared and ready for the chief judge, or did you go through any issues in collecting payment for our services. If none, type none and move on to next question.





OTHER INFORMATION
