General Information

Please provide all information requested so that we can update our files accordingly, and have the most current information for assignment sheets.


Availability

Put Y (Yes, I am available) or N (No, I am not available) next to each date. Marking that you are available does not guarantee we have work for you every weekend, but EVERY attempt is made to get all members as much work as is possible. Your submission of this availability form says you are available for and will accept assignments offered to you by the association on the dates you have marked as available for work. Should your availability change, you MUST immediately fill out and forward this form with your revised schedule. Failure to notify the association of a change in your schedule means you may be asked to work on a weekend you no longer have available, and adds to the work of the assistant director as they fill panels throughout the fall.


Affiliations

List all affiliations, as defined by the Red Book on page 18. For those who are prospectives, the definition of an affiliation reads as follows:

You are affiliated with a band or school if:

1. You have written any portion of their program in the past or current competitive season. This includes but is not limited to the musical score, drill, equipment work, choreography.

2. You have solicited products or services to the organization in the past or current competitive season. This includes but is not limited to fund raising events, tours, uniforms, equipment, instruments, program design services.

3. You are employed by the school district.

4. The organization has a competing members, employee, staff member, paid consultant, or unpaid volunteer who is also a member of your immediate family or a member of your household. Member of the immediate family are defined as: parents, siblings, spouse, children, grandchildren, in-laws. Household members are defined as anyone with whom you share a residence. This could also include children for whom you are not the parent or grandparent, but have been appointed legal guardian.

If your affiliation change after submitting this form, please submit a revision with updated information as mentioned above.






Certification

I provide this availability to the best of my knowledge at the time of submission. I will keep the Assistant Director notified of any changes to my availability and my affiliations to the best of my ability, and will follow the guidelines of the RED BOOK and of our association regarding availability, freelance judging assignments and acceptable judging locations. By submitting, I agree to the aforementioned guidelines and policies.
